Dear Peace Paddler:
We must have a completed and signed
Canoe Outpost-Peace River, Inc. Credit Card Authorization (CCA) on file at least
2 days before your paddle trip to hold your reservation. Nothing is charged on
the card unless canoes, kayaks, services, and/or camping gear are not used,
and/or damages and/or late fees are incurred, and/or search & rescue etc. If we
do not have your CCA by 4:00 p.m. two days prior to the trip, we will have to
automatically cancel your trip, you will not be contacted. Example: a Saturday
reservation needs a CCA on file by Wednesday at 4:00 p.m. to avoid cancellation.
We may contact you earlier for CCA if it is any extremely busy weekend and/or
holiday. Please complete and sign the
form and return to confirm your reservation. Form may be faxed or mailed,
contact information at bottom. Parties showing up but not taking all the
canoes reserved will be charged the quoted price plus sales tax per canoe/kayak
as per the CCA form. Personal canoes/kayaks/boats cannot be substituted for
rentals upon arrival. You do have the opportunity to make changes up to 48 hours
before the trip.
Credit Card Authorization Form (CCA)
We appreciate the decision you have
made to reserve and/or purchase products and/or services from Canoe
Outpost-Peace River, Inc. Since you have chosen the option of utilizing your
MasterCard, Visa, American Express, or Discover Card to reserve and/or pay for
your paddle trip, please complete the information below. The purpose of this
form is that due to banking regulations and as a result of banks suffering
extensive losses on credit card transactions, our bank has required that any
Canoe Outpost-Peace River, Inc. customer who wishes to use a credit card to
reserve/pay for rentals and/or services, must have a signature in our files
authorizing us to accept their credit card orders. This request will help to
protect you and your credit card account from any credit card misappropriations.
Please know that it is only necessary to complete this form once, for all of
your credit card orders with Canoe Outpost-Peace River, Inc. annually.
Please advise Canoe Outpost if you want
this form held for future use.
I authorize Canoe Outpost-Peace
River, Inc. to accept telephone, fax, e-mail, and mail orders. I have completed
the below Credit Card Authorization Form in good faith, and I understand that
any order/reservation is not valid until accepted by Canoe Outpost-Peace River,
Inc., who refuses the right to refuse any purchase orders, either written or
verbal. I understand that Canoe Outpost-Peace River, Inc. makes no warranties
other than those set forth in this agreement or created by law in the state in
which I reside.
Upon the completion and
signing of this form, I hereby authorize Canoe Outpost-Peace River, Inc. to
charge the below credit card the currently listed price and/or quoted price per
my reservation for each vessel reserved but not used on day of reservation if I
fail to notify Canoe Outpost-Peace River, Inc. of cancellation of any number of
reserved canoes/kayaks and/or outfitting. Cancellations must be made by the
prior day of services at noon to avoid being charged. I also authorize Canoe
Outpost-Peace River, Inc. to charge my account any service fees based on current
Canoe Outpost Service Fee Schedule (posted at main office for review or
available upon request) for any of the following that myself or others in my
party may incur: any lost and/or damaged equipment and/or damage to any physical
property owned or leased by Canoe Outpost-Peace River, Inc. or Peace River
Outpost, Inc, or Charlotte Bragg Trust; any late fees and/or for any search
and/or rescue of myself, anyone else in my party, and for any equipment;
for any use of Canoe Outpost equipment, facilities, and services such as
parking, docking, camping, and transportation of equipment and people that were
not previously reserved but incurred by myself or anyone else in my party that
were not part of the reservation, or not registered Canoe Outpost customers but
represented as members of my party; clean up of any picnic area, camping area,
beach area, or along the river and repair of any camping area and/or beach area
used by myself or others in my party, and for any preparations and
transportation of reserved camping gear.
Note: For any
challenges with mailed in or telephoned in credit card orders, an attempt to
reprocess the order will be made once each day for a period of not more than
three (3) consecutive business days using that same credit card number. Those
challenges may include: declined card, invalid card, illegible or incomplete
card number and/or expiration dates etc. Upon the third and final failed attempt
to process this order, the order will be deleted from Canoe Outpost-Peace River,
Inc.’s open order status. Although this Credit Card Authorization Form is on
file at Canoe Outpost-Peace River, Inc., Canoe Outpost-Peace River, Inc.
reserves the right to request additional information on each additional order.
The completion and delivery of this form do not constitute a reservation.
